    Thank you for your interest in participating in the 2025 Queen of Heaven Carnival. Submitting this application does not guarantee your acceptance, a representative from the Queen of Heaven Carnival Committee will follow up with additional information and next steps:
    Notes for accepted vendors:
    • All necessary insurance, health department permits, and all other required forms will be requested and required at the time of a contract agreement
    • Rent is determined by factors including location on the grounds and type of product / food sold
    • The Queen of Heaven Carnival is in contract for all vendors to provide Pepsi Products on grounds, more information will be provided at the time of contract agreement
